Consultation steps

The Ophthalmology Department of Créteil offers patients several types of consultations: General Ophthalmology, Age-Related Macular Degeneration (AMD), hereditary retinal degenerations and dystrophiesretinal venous occlusive disease, diabetic retinopathy, retinal inflammatory disease, myopic maculopathy, pediatric ophthalmology, retinal surgery, cataract surgery, glaucoma and eyelid pathology consultations.

Consultation

These different consultations (with the exception of Age Related Macular Degeneration consultations and rare diseases consultations) are organized using the same process.

After the patient is registered at the front desk, he or she is directed to the waiting room.

Ophtalmologic examination

Then is greeted by a doctor who performs an ophthalmologic examination: visual acuity is assessed, ocular pressure is measured, the anterior segment is examined, and the fundus is examined following dilatation of the pupil.

Complementary examinations

Following this examination, the doctor will often prescribe complementary exams such as a visual field exam, fluorescein angiography, optical coherence tomography (OCT), even OCT-Angiography, which facilitate accurate diagnosis and the choice of appropriate therapy. Some of these exams are performed the same day. Others require a follow up visit.

Medical retina

Consultations dedicated to Age-Related Macular Degeneration (AMD) and “Angiogenesis Consultations” have their own particular process.

Ophthalmologic examination

After registration with the front desk, the first step is the measurement of visual acuity which is performed with the help of a special scale called ETDRS, which is more precise in the setting of patients with retinal impairment. Initial consultations also include a full ophthalmologic exam including ocular pressure and evaluation of the anterior segment (cornea, lens in particular).

Complementary examinations

Once the pupils are dilated, additional complementary exams are performed selected based upon the particular elements of a patient’s dossier. Exams may include optical coherence tomography (OCT), fluorescein angiography, Indocyanine Green Angiography (ICG) and OCT-Angiography.

Following these procedures, the final step is fundoscopic examination followed by a discussion with the patient to select a treatment plan if necessary.

The standard treatment for this pathology is intravitreal injection (IVT). We have established a "One Day" patient's journey for IVT. This will lead to perform the injection the same day, without prior appointment, and to facilitate patient care.

IVT follow-up

For patients with exudative AMD (Wet AMD) for whom intraocular injection therapy is ongoing, a specific process is in place to reduce wait time and complete a follow-up appointment and injection on the same day.

These consultations occur on the first floor of Building B and resemble traditional AMD consultations: Visual acuity is measured followed by optical coherence tomography (OCT). These exams are typically sufficient to decide whether to continue or halt treatment, however additional exams (in particular, retinal angiography) are available if needed.

Once the follow up consultation has been conducted, the intraocular injection can be immediately performed. In some cases, the examination permits the delay of a scheduled injection.
